For access during regular business hours, please use the Intercom Access Point.

I understand that ALT Open access at Lake Church is granted for non-business hours only and is intended solely to support my approved ministry responsibilities. I understand that access during regular business hours must be requested and granted through the Intercom Access Point, and that this form may not be used for business-hours entry.

I agree to use ALT Open access responsibly and respectfully, in alignment with Lake Church policies, safety procedures, and facility guidelines. I understand that this access is not transferable, may not be shared with others, and does not grant permission to allow unauthorized individuals into Lake Church facilities.

I acknowledge that my access level is based on my current role and ministry needs at Lake Church and may be reviewed, modified, or revoked at any time, particularly if my role changes or if access is misused. I understand that misuse of access may result in immediate removal of access and further follow-up by Lake Church leadership.
